Who we are

Horizon Care QLD Pty Ltd (ABN 22 697 580 840) ("Horizon Care", "we", "us") is a Queensland-based provider of NDIS supports and Community Nursing, with Supported Independent Living and Aged Care Services in development. We are committed to protecting your privacy in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs) and, where applicable, the NDIS Practice Standards and Queensland health records legislation.

What information we collect

Depending on how you interact with us, we may collect:

  • Contact details — name, email address, phone number and postal address.
  • Enquiry details — the type of enquiry, your message and any information you choose to share with us through our website forms.
  • Health and disability information — only where you become (or are considering becoming) a participant or client, and only to the extent needed to plan and deliver safe care. This is "sensitive information" under the Privacy Act and is handled with additional care.
  • NDIS details — such as your NDIS number, plan dates and plan management arrangements, where relevant.
  • Website data — technical information such as browser type, pages visited and approximate location, collected through cookies and analytics tools.
  • AI assistant conversations — messages you send to the Horizon Care Assistant on our website, used to answer your questions and improve the service.

How we use your information

  • To respond to your enquiry, referral or expression of interest.
  • To assess, plan, deliver and review supports and clinical care.
  • To communicate with you, your nominated family members or carers, and your support coordinator or treating health professionals — with your consent.
  • To meet our legal, funding and reporting obligations, including to the NDIS Quality and Safeguards Commission and the NDIA.
  • To keep you informed about services you have registered interest in (for example, Aged Care Services or SIL). You can opt out at any time.
  • To improve our website and services.

Who we share it with

We do not sell your personal information. We share it only where necessary to provide our services or as required by law — for example with your treating GP or allied health team (with consent), the NDIA or NDIS Commission, our professional advisers, and trusted service providers who host our website, send our emails or store our data. These providers are bound by confidentiality and privacy obligations.

How we store and protect it

Your information is stored on secure, access-controlled systems. Enquiry data submitted through this website is stored in encrypted databases and transmitted over HTTPS. We retain personal information only for as long as needed for the purposes above and to meet record-keeping obligations, after which it is securely destroyed or de-identified. Some of our service providers may store data outside Australia; where this occurs we take reasonable steps to ensure it is protected to Australian standards.

Cookies and analytics

Our website uses essential cookies to function and may use analytics cookies to understand how visitors use the site. You can control cookies through your browser settings; disabling some cookies may affect how the site works.

Your rights

You may request access to, or correction of, the personal information we hold about you at any time. You may also withdraw consent, ask us to stop contacting you about future services, or make a complaint about how we have handled your information. We will acknowledge complaints promptly and aim to resolve them within 30 days. If you are not satisfied with our response, you may contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.

We acknowledge the Traditional Custodians of the lands on which we live and work across South East Queensland, including the Yugambeh, Turrbal, Jagera and Quandamooka peoples. We pay our respects to Elders past, present and emerging, and recognise their continuing connection to land, waters and community.

We also honour all who have helped build this great country — the pioneers, explorers and settlers, the migrants who made Australia home, those who have served, and the families who carry their stories forward. Whatever your background, you are welcome at Horizon Care.
