This website is operated by Horizon Care QLD Pty Ltd (ABN 22 697 580 840). By accessing or using horizoncareqld.com you agree to these Terms of Use. If you do not agree, please do not use the site. Service agreements for the delivery of supports are separate documents and take precedence over these website terms.

General information only

Content on this website — including pages about the NDIS, our services, blog articles and responses from the Horizon Care Assistant — is provided for general information only. It is not medical, clinical, legal or financial advice and does not create a provider–participant relationship. Always seek advice from a qualified health professional or the NDIA about your individual circumstances. In an emergency, call 000.

Our registration status

Horizon Care QLD has completed its Stage 1 and Stage 2 NDIS audits and is awaiting formal registration by the NDIS Quality and Safeguards Commission. Until registration is granted, we can support self-managed and plan-managed participants only. Services described as "coming soon" (including Supported Independent Living and Aged Care Services) are not yet available and are subject to the relevant approvals. Registering your interest does not create any obligation on you or on us.

Horizon Care Assistant

The Horizon Care Assistant is an automated AI tool that answers questions about our organisation and the NDIS. It may make mistakes, cannot confirm service availability or pricing, and does not provide clinical advice. Any commitment to deliver services can only be made by a member of our team in writing.

Using this website

• You must not use the site for any unlawful purpose, to submit false or misleading information, or to interfere with its security or operation.
• Information you submit through our forms must be accurate and must only relate to you or to a person you are authorised to act for.
• We may suspend or restrict access to the site at any time for maintenance, security or any other reason.

All content on this website — including text, graphics, logos and the Horizon Care brand — is owned by or licensed to Horizon Care QLD Pty Ltd and protected by Australian copyright and trade mark law. You may view and print content for personal, non-commercial use only. Third-party trade marks (including the NDIS logo) remain the property of their owners.

Our site may link to external websites such as ndis.gov.au. We do not control and are not responsible for the content or privacy practices of those sites.
